Plot
Synopsis
The attendings vie to be Chief. Two people who haven't slept together before, will. McSteamy's 60-day vow of chastity has a conclusion. We meet a boss, his assistant, and his wife. We also get to see someone being treated in the clinic. Derek has more questions about his relationship with Meredith. The chairman of the hospital board is admitted as patient. Callie confronts Izzie about her relationship with George.

The interns are studying and stressing out for their intern exams. Preston tries to distract Cristina with tasting wedding cake, while the other interns accuse George of studying secretly with Callie's legendary flashcards, and hoarding them for himself. Mark is eagerly counting down the days until his Addison-mandated break from sex is over, but Addison doesn't have the same level of anticipation. Meredith thinks Derek is avoiding her because she told him she needs time to study, but she wants more attention from him. Cristina is growing rapidly irritated with Burke's wedding obsession; he brings more samples of wedding cake to work. Bailey assigns the interns to light duty, so they can study for their intern exams.
The chairman of the board is admitted to the hospital for a discreet procedure, cutting his vacation in the Amazon short. The Chief wants Bailey to devote her sole attention to the case, which frustrates her. She jumps back and forth between cases anyway. The attendings are eager to jump on the case and possibly catch the chairman's attention. The problem is of a delicate nature—his testicles are enlarged to the size of grapefruits, and he hasn't urinated in three days. His assistant reveals that she is sleeping with the Chairman, and worried that she also has whatever disease he has contracted. She also says that they are in love. The Chairman lambasts the Chief, saying that he wants the Chief to be taking care of his case—and that he is disappointed in the Chief for retiring, and for the Chief's loss of passion and commitment to his job. He says the Chief delegates too much.
Ava is over her anger at Alex, but he isn't so quick to warm up to her after she lashed out before. She tells Alex about Addison and Mark's sex bet, which she overheard from the nurse's station, and encourages Alex to go after Addison. Ava's blood pressure is very high, so Alex goes to tell Addison, who is enjoying Burke's wedding cake samples. Alex reveals through a typical snarky comment that he knows about her bet with Mark.
The situation between George and Izzie is still awkward. Izzie resents having to work with Sidney, who is irritated with what she perceives as Bailey's power play to get Chief Resident. Their turf war continues. Thanks to Sidney's insistence on treating a patient who only has the sniffles, they discover that his brain is pressing into his skull—causing spinal fluid to leak out through his nose.
Cristina tries to suck up to Callie in hopes of getting her cards, and Callie brushes her off. She confides in Addison about how she is disappointed with George for lying to her the other day about Izzie. Addison tells Callie that even though she cares for Mark, he's not the kind of person she wants to spend her life with—she can't see herself having a family with him, for example. Callie encourages Addison to get with Alex.
The Chairman's problem is revealed to be an exotic fish that swam into his penis and currently is in his bladder (referring to the Candiru). Meredith says that it's karma. Cristina says she cheated on all of her boyfriends. George and Izzie eat wedding cake while Callie jealously watches. The Chairman's wife reveals that she knows he is having an affair with his assistant. Both the wife and the assistant leave him.
Ava starts having seizures, and they perform a C-section on her. Addison gets angry with Alex for being so attentive to Ava's case, and accuses him of developing feelings for her. Alex tells her that the reason he's so attentive is because he wants to be prepared out of respect to Addison. Addison and Alex have sex in the on-call room. Ava tells Alex that he should go for it with Addison, that he's a good guy underneath even if he acts like a jerk.
Cristina continues to harass Callie for the cards. Callie gives them to her out of irritation with George. Callie confronts Izzie in the elevator, saying that she knows it's not Izzie's fault, but that she is jealous of the bond that George and Izzie share and tired of trying to compete with Izzie for George's attention. She asks Izzie to stay away from George. Izzie tells George that she needs space.
Mark figures out that Addison and Alex had sex. He propositions Meredith, who rebuffs him. Meredith goes to Derek's trailer. Derek is ambivalent about his relationship with Meredith, saying that he knows she didn't try to swim when she was drowning and that he can't be responsible for her mental health and functioning.
The chief criticizes Bailey for her lack of ability to delegate, and warns her that she could lose her family unless she learns to balance her life.
Izzie confides in Preston, who tells her that marriage is important. She tells George that she can't understand how they can pretend like there's nothing between them. George tells her he is transferring to Mercy West.
Addison invites Mark out, and he lies to her and tells her that he slept with someone, giving her an easy out from their bet. Addison, upset but relieved about Mark's breaking the bet, goes to Alex and offers to help him study. He tells her he doesn't want a commitment with her and doesn't have time for a relationship. She leaves, pretending not to be upset.
