Medical record mix up

A medical record mix up (also known as a medical flaw, or medical screw up for slang) is when a nurse or anyone working in a doctor's office mixes up a patient's medical files and/or records. For instance, suppose a patient named John Smith comes in for blood testing. The secretary gets a hold of his records, however, these records are for a different John Smith. This is common in medical facilities, and can be considered unprofessional and dangerous towards the patient.
