It runs from an intersection with County Route 57 (former NY 57) in the village of Phoenix north to a junction with NY 3 in the town of Palermo. The southernmost portion of NY 264 serves as a connector between Phoenix and NY 481, which bypasses the village to the northeast.

Route description
NY 264 begins at an intersection with County Route 57[4] (formerly NY 57)[5] in the small village of Phoenix. The route heads north as Volney Street through the village and into the surrounding town of Schroeppel, where the highway becomes known simply by its numerical designation. NY 264 continues onward through a lightly populated area to an interchange with NY 481 roughly 1.5 miles (2.4 km) north of Phoenix.[4]
North of NY 481, the route passes through more rural areas of Oswego County. NY 264 heads though cultivated fields and forested areas into the town of Palermo, where it intersects NY 49 in the community of Suttons Corner. The route comes to an end roughly 2 miles (3.2 km) to the north at a junction with NY 3.[4]

NY 264 was assigned ca. 1932 and has not been altered since.[1][2]
