FSN Midwest is a regional sports network that operates in eastern and central Missouri, Southern Illinois, Southern Indiana, and Iowa. FSN Midwest began in the November 1989 as Prime Sports Midwest as an affiliate of the Prime Network, which then was seen mainly in Indiana, before moving westward in 1994. It joined Fox Sports Net and changed its name to Fox Sports Midwest on November 1, 1996, then to Fox Sports Net Midwest in 1999, before going with its current name in July 2004.

In early 2008, as part of a deal to return Kansas City Royals to FSN, FSN Kansas City was spun-off FSN Midwest as the new FSN affiliate for the Kansas City market.
