Plot

As Elliot begins to plan her wedding with Keith, J.D. continues to question his true feelings for her. In an attempt to ease his best friend's mind, Turk takes him to a medical convention in Phoenix. The two have a good time looking at the new gadgets (especially the Dr. Toilet) until J.D. encounters his ex-girlfriend Kim and discovers that she is still pregnant with his child and had lied to him about having a miscarriage. Kim begs J.D. to wait for her at the hotel bar so she can fully explain herself, but J.D. leaves without hearing her out.

Back at Sacred Heart, Dr. Cox talks to Elliot about why she chose to marry Keith, and she starts contemplating whether she is getting married because she loves Keith, or because she just wants to get married. Meanwhile, Dr. Kelso and his cronies (who are also chiefs of medicine) party it up at the convention while unknown to him, the Janitor takes on the role of chief of medicine back at Sacred Heart. The Janitor unsuccessfully tries to pin the blame on Ted.

The episode ends with J.D. returning to Sacred Heart. Elliot immediately asks him whether or not he feels that her marrying Keith is a mistake. At the same time, Kim arrives at Sacred Heart, seeking to make amends and get J.D.'s support.
